MyHealth Care Centre respects your privacy. This policy explains, in plain language, what personal information we collect, why, how we protect it, and your rights. As a health care provider in Ontario, we follow the Personal Health Information Protection Act, 2004 (PHIPA).

1. What we collect

  • Your name, date of birth and contact details (phone, email, address).
  • Appointment details, such as dates, times and the practitioner you see.
  • Health information you share with us or that we record during your care, including your health history, assessments and treatment notes.
  • Billing and insurance information needed to process payments and claims.
  • Messages, call notes and requests you send us by phone, text, email or our website.

2. How we use it

  • To book, confirm, change and remind you about appointments.
  • To provide and plan your care.
  • To bill you or your insurer and keep accurate financial records, including in our accounting software (QuickBooks Online).
  • To answer your questions and meet our legal and professional obligations.

Our accounting records contain only what is needed for billing and bookkeeping. We do not put your health notes into our accounting software.

3. We do not sell your information

We never sell, rent or trade your personal information. We share it only with your consent, with service providers who help us run the clinic (for example our booking, phone, email and accounting systems) under confidentiality obligations, or when the law requires or allows it.

4. How we store and protect it

Your information is kept in secure, access-controlled systems. Only staff and providers who need it for your care or for clinic operations can see it. We use passwords, encryption where available, and secure connections, and we keep records only as long as required by law and our professional college.

5. Your rights

You may ask to see your personal health information, ask us to correct it if it is wrong, or withdraw consent for certain uses, subject to legal limits. Send your request to us using the contact details below and we will respond within the time PHIPA requires. If you are not satisfied with our response, you can contact the Information and Privacy Commissioner of Ontario.
